In a new study, cognitive decline in dogs is linked to a shorter stride length—but only in their front limbs. The finding offers a non-invasive early marker for canine dementia and echoes the brain-body connection seen in humans.
The Research
Researchers at North Carolina State University, led by Natasha Olby, professor of neurology and the Dr. Kady M. Gjessing and Rhanna M. Davidson Distinguished Chair in Gerontology at the College of Veterinary Medicine, tracked 88 geriatric dogs with an average age of about 12 years over multiple six-month intervals. The study, published in a peer-reviewed journal, aimed to see if the same gait changes that signal cognitive impairment in humans also appear in dogs.
Each dog underwent physical, neurologic, and orthopedic exams, mobility assessments, hearing tests, and blood work. Owners completed standardized questionnaires, including the Canine Dementia Scale (CADES) and the Canine Brief Pain Inventory (CBPI). For gait analysis, trained observers measured stride length as dogs walked a straight, five-meter indoor walkway. Stride length was recorded separately for front (thoracic) and back (pelvic) limbs, then adjusted for the dog’s height.
The results were striking: higher CADES scores—indicating worse cognitive decline—were significantly associated with shorter front limb stride length, even after adjusting for age and pain scores. Specifically, every 10-point increase on the CADES corresponded to about a 1.2% reduction in thoracic limb stride length. Hind limb stride length showed no such correlation.
Why the difference? “While thoracic limbs play a key role in braking and postural stabilization, pelvic limbs mainly act as a propulsion motor,” Olby explains. “Thoracic limb movement is likely under more cortical influence.” In other words, front limbs require more brainpower for balance and spatial awareness, making them more sensitive to cognitive changes.

This research highlights a powerful principle: the brain and body are deeply connected. Just as walking speed and stride length can be early signs of cognitive impairment in humans, subtle changes in how a dog moves may reveal underlying brain decline. For pet owners, this means that monitoring gait—especially front-limb stride—could help detect canine dementia earlier, allowing for better veterinary care and monitoring. More broadly, it reinforces that motor control, particularly of limbs under heavy cortical influence, is a window into brain health.
