New AI Model Predicts Alzheimer's Progression Without Costly Scans

Alzheimer's disease progresses differently in each person, making it hard for doctors to predict what comes next. Now, an AI tool called GNOVA can reconstruct past and predict future cognitive decline using only routine health data, bypassing expensive brain scans and spinal taps.

The research

Researchers from the Indian Institute of Technology Delhi and collaborators trained GNOVA on data from 1,727 Alzheimer's patients in the Alzheimer's Disease Neuroimaging Initiative (ADNI) over up to 10 years. They published their findings in June 2026 on the preprint server arXiv.

GNOVA stands for GRU-Neural ODE Variational Autoencoder. It combines a GRU (Gated Recurrent Unit) encoder with a Neural ODE (Ordinary Differential Equation) decoder inside a variational autoencoder. The GRU encoder handles irregularly timed visits, while the Neural ODE allows continuous interpolation and extrapolation at any time point. The variational autoencoder provides uncertainty estimates for each prediction.

The model predicted two standard cognitive scores: CDR-SB (Clinical Dementia Rating Sum of Boxes) and MMSE (Mini-Mental State Examination). It achieved a mean absolute error of 1.35 for CDR-SB and 2.28 for MMSE, all without using neuroimaging or biomarker data.

Feature-ablation studies showed that age, body mass index (BMI), and the presence of the APOE4 gene variant were the strongest predictors. This means that in resource-limited settings, clinicians can still make reliable predictions with basic patient information.

Why it matters

Most existing models only forecast future cognitive state. GNOVA can also reconstruct past missed visits, giving a fuller picture of the disease trajectory. For patients who miss appointments or only start visiting late, this could help doctors fill in gaps and plan care more accurately.

Moreover, because GNOVA does not rely on expensive biomarkers or imaging, it opens the door to better Alzheimer's management in low-resource settings around the world.

What you can do

While this research is not yet a clinical tool, it highlights that simple health metrics can be powerful. Stay on top of your regular check-ups, keep track of your numbers like BMI and blood pressure, and if you carry the APOE4 gene, know that your doctor can still monitor you with simple tests. For your own brain health, mental exercise and physical activity remain the best known protections.
