Ketamine and psychedelic drugs may lift depression by changing how the immune system talks to the brain, according to a new study. Researchers found that these distinct treatments converge on the same immune signals, offering a potential blood test to predict who will benefit.
The Research
Scientists at the University of Texas MD Anderson Cancer Center led the study, published in Molecular Psychiatry. They analyzed laboratory models and a previous clinical trial of ketamine for treatment-resistant depression. The team discovered that despite acting on different receptors in the brain—ketamine on NMDA receptors and psychedelics like psilocybin on serotonin receptors—both trigger similar changes in immune-related molecules in brain cells. In particular, they zeroed in on two cytokines: interleukin-15 (IL-15) and interleukin-7 (IL-7). These immune signaling molecules were altered in the same way after treatment.
In the clinical trial, patients who responded well to ketamine had distinct baseline immune profiles: lower IL-15 activity and higher B-cell signaling compared to non-responders. After successful treatment, these imbalances were reversed, and patients' brain electrical activity (measured by EEG) shifted correspondingly. The blood-based biomarkers matched changes in the brain, suggesting that a simple blood test could serve as a proxy for what's happening in the brain.

Depression is a major cause of disability worldwide, and many people don't improve with standard antidepressants. Rapid-acting treatments like ketamine and psychedelics can work quickly, but doctors can't always predict who will respond. If these immune biomarkers are validated, they could guide treatment decisions and spare patients from trying ineffective therapies. Moreover, understanding this neuroimmune axis could lead to new drug targets that are more accessible than psychedelics or ketamine.
