Brain Aging Accelerates in Alzheimer’s, Addiction, and Psychiatric Disorders, Study Maps Regional Signatures

A new study maps how different brain conditions accelerate aging in specific regions. Alzheimer’s disease and mild cognitive impairment (MCI) show the highest overall brain aging, followed by psychiatric disorders and substance addictions. But ADHD and autism spectrum disorder? No significant acceleration at all.

The Research

Led by Shile Qi from Nanjing University of Aeronautics and Astronautics, the team analyzed structural MRI scans from 45,900 healthy controls and 2,698 individuals across nine conditions. They used a metric called Predictive Age Difference (PAD) — the gap between chronological age and age predicted by brain structure. A positive PAD means the brain looks older than expected.

The results, published July 21 in PLOS Medicine, rank conditions by PAD: Alzheimer’s disease and MCI top the list, followed by alcohol/tobacco addiction and psychiatric disorders (schizophrenia, bipolar disorder, major depressive disorder). Intriguingly, ADHD and autism showed no PAD increase, suggesting neurodivergence itself does not equal accelerated brain aging.

Regional patterns were distinct: the prefrontal cortex showed elevated PAD across many disorders. Psychiatric conditions specifically affected frontal and temporal lobes, while dementia impacted frontal and occipital cortex. Addiction selectively accelerated aging in the default mode network, salience network, putamen, and thalamus.

The team also linked regional PAD maps to condition-specific gene expression patterns, providing biological insight into the molecular pathways behind accelerated decline.

Why It Matters

These findings offer potential biomarkers for early detection and personalized monitoring. If your clinician can spot which brain regions are aging faster, they may tailor interventions earlier. For the curious mind: it reinforces that not all cognitive differences reflect pathology — ADHD and autism are not synonymous with 'older brains.' Acceleration signatures are condition-specific, not universal.
