High Cognitive Scores Linked to Higher Depression Relapse Risk

A surprising new study suggests that for people with a history of depression, having sharper cognitive skills might actually increase the risk of a relapse—the opposite of what happens in people without that history.

The Research

Researchers from the University of Birmingham and the University of Oxford analyzed data from 1,800 UK participants from the UK Biobank. Half had experienced at least one depressive episode in the past, while the other half were healthy controls matched by age and sex. All participants completed a battery of cognitive tests—including reaction time, memory, and word-pairing games—and underwent brain scans.

The team expected that "brain fog"—the memory and concentration problems that affect up to 90% of people with major depressive disorder (MDD)—would predict future depressive episodes in both groups. And indeed, among the healthy controls, lower cognitive scores were linked to a greater chance of experiencing a first depressive episode during the follow-up period.

But the results for those with a history of depression were the opposite: 33% of them relapsed, while only 13% of controls had a first episode. Crucially, among those with a history, participants with the highest cognitive scores were the most likely to relapse, while those with lower scores were less likely. This "paradox" confounded expectations.

"We expected poorer cognitive performance to be associated with a higher risk of future depression in both groups," said Dr. Angharad de Cates, lead author from the University of Birmingham. "But the results were more nuanced than expected. Among people with previous depression, those with higher cognitive scores were more likely to experience a further depressive episode than those with lower scores—the opposite pattern to that observed in controls."

Why It Matters

This finding challenges a common assumption that cognitive impairment is always a risk factor for worse mental health. It suggests that the relationship between cognition and mood is complex and context-dependent. For some people, higher cognitive functioning might enable them to recognize their symptoms and seek help—or it might expose them to different social pressures and self-awareness that contribute to relapse. Understanding this could help clinicians better predict who might be at risk, and tailor preventive strategies.

If you have a history of depression, don't assume that feeling sharp means you're in the clear. Monitor your mood and stress levels regularly, and seek support if you notice early signs of relapse—even if you're thinking clearly. For everyone, maintaining good cognitive health through sleep, exercise, and mental challenges remains important, but it's only one piece of the mental health puzzle.
