Living in a socioeconomically disadvantaged neighborhood is associated with a brain that looks older than its years, according to a large clinical MRI study published in Radiology.
The Research
Researchers at the University of Wisconsin School of Medicine and Public Health analyzed 2,826 clinical brain MRI scans (1,094 men and 1,732 women, ages 18 to 96) collected between January and June 2024 at University of Wisconsin Hospitals and Clinics and community healthcare partners. Led by senior author John-Paul J. Yu, M.D., Ph.D., the team excluded scans showing visible neurological disease, normalized brain volume to head size, and controlled for age and sex.
To capture environment, they mapped each patient's ZIP code to the Area Deprivation Index (ADI), a census block–level measure built from 17 markers of income, housing quality, employment, and education drawn from the 2023 American Community Survey.
Two clear patterns emerged. First, people in the most deprived neighborhoods had a larger "brain age gap" — their structural MRI looked biologically older than their chronological age. Second, they showed lower total brain volume, a sign of global tissue loss. The team also found higher white matter hyperintensity volume, bright lesions on MRI that reflect ischemic injury and microvascular deterioration.
"We found that residents in the most disadvantaged neighborhoods had an accelerated brain age compared with people living in less deprived areas," Yu said. "In other words, where you live leaves a measurable imprint on your brain."
Why It Matters
A higher brain age gap has been tied to greater susceptibility to cardiovascular disease, cognitive impairment, and psychiatric disorders. White matter hyperintensities are what Yu calls "the footprints of cardiovascular damage in the brain." Together, these markers suggest that structural inequities don't just affect opportunity — they leave measurable biological marks on the brain at cellular and molecular levels.
