Brain's Hidden Wiring May Protect Cognition as Gray Matter Shrinks

A new brain imaging study suggests that healthy wiring just beneath the brain's gray matter may help cushion the effects of age-related brain loss, particularly for language skills.

The Research

Scientists at the Mark and Mary Stevens Neuroimaging and Informatics Institute (Stevens INI) at the Keck School of Medicine of USC examined brain scans and cognitive test results from 459 adults aged 60 and older living in communities across India. The study, published in Alzheimer's & Dementia: The Journal of the Alzheimer's Association, is among the first to investigate superficial white matter in a community-based population from a low- and middle-income country.

Superficial white matter forms a thin layer of short, curved nerve fibers directly beneath the gray matter on the brain's outer surface. These fibers connect nearby regions of the cerebral cortex—think of them as local roads linking neighboring neighborhoods. Gray matter, by contrast, contains the nerve cells that process information. Using advanced diffusion MRI, which tracks water movement through brain tissue, researchers measured neurite density and free water content to assess the health of these local connections.

Participants also completed cognitive assessments covering language, memory, executive function, and visuospatial ability. The most consistent link between superficial white matter health and cognition involved language. People with healthier superficial white matter performed better on language tests, with the strongest associations in frontotemporal regions involved in recognizing words, speaking fluently, and holding language information in mind.

Gray matter atrophy remained the strongest overall predictor of cognitive ability. But the impact of gray matter loss varied depending on the health of nearby superficial white matter. When local connections showed poorer integrity, gray matter loss was more strongly tied to impaired language and broader cognitive difficulties. When superficial white matter was healthier, that link was weaker.

"The findings point to superficial white matter as a possible source of resilience," said Leon Aksman, PhD, assistant professor of research neurology at the Stevens INI and senior author of the study.

Why It Matters

This research offers a possible explanation for why two people with similar amounts of age-related gray matter loss can experience very different levels of cognitive decline. It suggests that cognitive health depends not only on how much gray matter is preserved, but also on the condition of the wiring that connects it. For anyone concerned about brain aging, this shifts attention toward the health of communication pathways—not just brain volume alone.

While this study doesn't prove that improving superficial white matter will prevent cognitive decline, it points to general brain-health habits that support white matter integrity. Staying physically active, managing blood pressure and blood sugar, getting enough sleep, and challenging your brain with new learning may all help maintain healthy neural connections. Consider adding a daily language-based activity—reading aloud, learning new vocabulary, or doing crossword puzzles—since language showed the strongest link in this study.
