Depression in older adults is linked to a smaller volume in a specific part of the brain's memory center, the hippocampus, according to a new study. The reduction was found in a region called CA23DG, which is crucial for distinguishing similar experiences and retrieving memories, and was independent of Alzheimer's disease markers.
The Research
Researchers from the USC Mark and Mary Stevens Neuroimaging and Informatics Institute examined high-resolution MRI scans from 2,009 cognitively healthy adults aged 50 to 90. Of these, 630 had depression, while 1,379 did not. The study, published in Translational Psychiatry, focused on three hippocampal subfields: CA1, subiculum, and CA23DG (which includes CA2, CA3, and dentate gyrus).
The team found that participants with depression had significantly smaller volumes in the CA23DG subfield, but no significant differences in the CA1 or subiculum. This association held even after accounting for body mass index, physical activity, and key Alzheimer's biomarkers such as amyloid-beta, tau proteins, and the APOE ε4 genetic variant.
“Depression has long been associated with an increased risk of developing Alzheimer’s disease, but we still do not fully understand the biological connection,” said lead author Danielle Luu, a PhD student at USC. “By looking closely at the individual parts of the hippocampus, we identified a specific area that may be particularly sensitive to depression in older adults.”
The study also observed that depressed participants using antidepressants had smaller CA1 and CA23DG volumes than non-medicated depressed peers, but the authors caution this likely reflects greater disease severity or duration rather than medication toxicity.

This finding highlights that depression may have a targeted effect on brain structure, potentially explaining the increased risk of cognitive decline and Alzheimer's disease seen in depressed individuals. The CA23DG region is essential for pattern separation (telling similar experiences apart) and pattern completion (reconstructing memories from partial cues), which are often impaired in aging and dementia.
