Brain's Waste-Clearing System Has Fast and Slow Lanes

Artificial intelligence has helped scientists map the brain's hidden cleaning system, revealing two distinct speeds for fluid flow: a fast lane around the brain's outer surfaces and a much slower lane through deep tissue. This discovery could lead to earlier detection of circulation problems linked to Alzheimer's, aging, and concussions.

The Research

Researchers at the University of Rochester, Brown University, and the University of Copenhagen used a physics-informed AI approach to calculate fluid flow speeds from MRI data. Led by Professor Douglas Kelley of URochester's Department of Mechanical Engineering, the team trained neural networks on videos showing dye spreading through brain tissue over time. By analyzing the dye's movement, the AI estimated both the fluid speed and the permeability of surrounding tissue.

The findings, published in Science Advances, show that in open areas around the brain—like the space between the skull and brain—fluid travels at a few microns per second. But deeper inside brain tissue, the flow is about 50 times slower. This dual-speed system is part of the glymphatic system, first described in 2012 by Maiken Nedergaard, co-director of the URochester Center for Translational Neuromedicine.

“You can put a microscope on a small patch of the brain and watch what's happening there with a lot of detail,” says Kelley, “but it's only a tiny view of the overall process. MRI gives a 3D view but does not capture flow velocity for flows this slow.” The AI overcame that limitation.

Why It Matters

This research could help clinicians detect circulation problems in the brain early. “We hope to someday be able to see whether an Alzheimer's patient has poor circulation in their brain or even screen for poor circulation earlier in life to try to stave off Alzheimer's,” says Kelley. “Or we could check when somebody has been concussed to see whether the fluid circulation in their brain is disrupted.”
