Memory errors and a loss of motivation may share the same broken brain circuit, according to a new study on autoimmune limbic encephalitis — a condition in which the immune system attacks the brain's memory hub, the hippocampus.
The research
Bahaaeddin Attaallah of Imperial College London and Maria Raquel Maio of the University of Oxford led a study comparing patients with autoimmune limbic encephalitis against age-matched healthy controls. The work was published in the Journal of Neuroscience, with the source material shared via the Society for Neuroscience and Neuroscience News.
Participants completed spatial memory tasks, specifically object-location binding — pairing specific objects with the exact places they appeared. Patients performed significantly worse than controls on these tasks, a deficit that directly reflects hippocampal damage. The findings were then examined using structural and functional neuroimaging to map what was happening inside the brain.
The key result was a correlation exclusive to the patient group: the more object-location errors a person made, the more severe their clinical apathy. Apathy here means a loss of goal-directed motivation, initiative, and emotional responsiveness — not sadness or despair. Crucially, this link held even after researchers accounted for depressive symptoms and general cognitive ability, suggesting apathy in these patients is biologically distinct from depression.
Imaging revealed two neural markers behind the pattern. First, patients who made more memory errors had smaller hippocampi — measurable atrophy in the structure. Second, those same patients showed weaker functional and structural connectivity between the hippocampus and the prefrontal cortex, the brain's hub for executive control and motivation. This coupling between memory errors and apathy was observed only in patients, not in healthy controls.
Attaallah said the team is now investigating the apathy-cognition link in different cohorts: "We are investigating the link between apathy and cognition in different cohorts with Parkinson's disease and dementia using different methods, including large data analytics and experimental tasks. This will allow us to have a deeper understanding of how human memory and motivation overlap in these diseases."
Why it matters
Memory loss and apathy often appear together in neurodegenerative conditions like Alzheimer's disease and Parkinson's disease, but the reason has been unclear. This study points to a shared circuit — the pathway between the hippocampus and prefrontal cortex — that may underlie both symptoms. That reframes apathy not as a psychological reaction to forgetting, but as a signal that the same neural wiring supporting memory may also support motivation.
