Wearable BCI Detects Hidden Consciousness in Brain-Injured Patients

In a groundbreaking study, researchers have used a wearable brain-computer interface (BCI) headset to detect hidden consciousness in patients with severe brain injuries—nearly doubling the detection rate from 39% to 69%.

Published in Communications Medicine by scientists at the University of Bath (UK), the study examined 42 patients with prolonged disorders of consciousness (PDoC) or locked-in syndrome (LIS). These conditions leave individuals aware but unable to move or speak, often leading to misdiagnosis as unconscious. Standard bedside tests rely on physical responses like eye tracking or flinching, but up to 40% of minimally conscious patients are mistakenly classified as unaware.

The team developed a lightweight, portable EEG headset that records brain activity while patients imagine simple motor actions—like lifting their left hand or both feet. A key innovation: instead of a single test session, patients underwent multiple training sessions with real-time auditory feedback. When the BCI detected the intended brain signal, a sound confirmed success. This neurofeedback allowed patients to refine their mental strategies over time, making their brain waves distinct and consistent. On average, the multi-session framework boosted detection of covert consciousness from 39% (using traditional behavioral checklists alone) to 69%. Among those who showed reliable brainwave modulation, 90% advanced to a staged questioning phase where they used different motor imagery to answer basic yes/no questions.
