A new AI model can predict long-term health risks—including heart disease, cognitive decline, and even double the risk of death—by mining hidden signals from routine sleep tests. This breakthrough, published in Nature Communications, suggests that the sleep data we already collect holds far more insight than we've been using.
The Research
Led by a multidisciplinary team from Cleveland Clinic, IBM, and Yale School of Medicine, researchers analyzed data from the Cleveland Clinic's STARLIT registry—a massive collection of sleep study records. The AI model was trained on full polysomnography signals, which capture brain waves, breathing, muscle activity, and heart rhythms during an overnight sleep test.
The AI sorted patients into five distinct risk subtypes. Patients in the highest-risk group had a 100% increase in five-year mortality risk compared to the lowest-risk group—a difference that the standard apnea-hypopnea index (AHI) completely missed. The model also predicted outcomes equally well for men and women, whereas AHI has historically been more accurate in men.
The study included data from the STARLIT registry and was independently confirmed in a nationwide cohort. Each year, an estimated 1 to 4 million polysomnograms are performed in the U.S., primarily to diagnose sleep apnea, yet clinicians typically focus on only a few summary numbers. This study shows that these routine tests contain a goldmine of physiological information that AI can unlock.
